Below, please find a link to an on-line
There are three versions of this survey: for district staff, for parents/community members, and for students. As a parent/community member, we ask that you complete the one referenced herein.
Here are six important instructions for the on-line survey:
1. This survey will be open (meaning available online) until 5 p.m. Monday, November 30.
2. Once you start the survey, you cannot leave the link and then go back -- although if you would like to change an answer, you can go back and forth from page to page while you are in the survey.
3. All answers will go directly to Patron Insight and the district will get a summary report. (Patron Insight, Inc. is the company that created the survey after talking with focus groups of parents, board members, and district staff.)
4. By design, the system will only allow one response per computer.
5. Printed copies are available from the CAO, if you
would prefer to fill out a survey in that format, or if you don’t have
access to a computer that hasn’t been used to complete a survey. Just call the
central administrative office at 319-688-1000 to request that one be mailed to
you. To return your completed hard copy survey, you may mail it back to the
