As time goes on, we hope to make more things available here. Any suggestions about a school-related form or document that you would like to see can be sent here.

Children’s Hospital of Iowa is proud to offer the Better Eating, Stronger
Teens (BEST) program.
This new program is for 7-12th grade girls who are
overweight or at risk for overweight (BMI >85%ile).
Master Schedule: This document lists each faculty member, the classes they are teaching each trimester and the periods each class meets.
Math Courses Sequence Flowchart: This document is a flowchart meant to show graphically the sequence that students typically follow depending upon the math track that they are in.
Non-Prescription Medicine: Before any non-prescription medication can be dispensed by the health associate, this form must be completed and be on file in the health office.
Permission to Practice Form: This must be on file with the office before a student may practice with any sports team.
Photo and Video Parent Release Form
Physical Examination Form: Before participating on any extra-curricular athletic team, a student must have on file this form.
Prescription Medicine: Before any medication prescribed by a doctor can be dispensed by the health associate, this form must be completed and be on file in the health office.
Recommended School Supplies: A listing of what various classes recommend student's have for materials

Standardized Grading Percentages and Letter Grade Scale
Weekly Report Form: This is a simple form used by some parents and students to check on a student's status in classes at the end of each week.
